Acne scars are a very common concern among many people.
What are acne scars?
Acne scars are irregularities in the texture of the skin secondary to acne. They are usually what is called atrophic scars. This means that there is an indentation in the skin where the scar is. This is in contrast to hypertrophic scars which are elevated from the skin. Sometimes after a pimple or blemish goes away, a dark spot is left on the skin. These are not actually acne scars, there are discolorations called PIH (post inflammatory hyperpigmentation). This is darkening of the skin that occurs after any type of inflammation. The inflammation can be due to acne, sun exposure, cuts, burns, laser treatments, of many other things. Unlike acne scars (which are textural changes in the skin) PIH is treated with products like the Brightening Cream, Exfoliating Serum, and Triple Antioxidant Serum and treatments such as the Forever Young BBL IPL photofacial treatment and the Soft and Smooth Rejuvenation treatment.
What causes acne scars?
True acne scars are caused by picking at or popping pimples. This leads to release of the pus into the skin causing an inflammatory response which damages the collagen. Collagen is the protein in the skin that keeps the skin smooth and tight. When the collagen is damaged acne scars form.
How are acne scars treated?
In order to treat acne scars we need to create new, fresh, and healthy collagen. Products that contain retin-A such as the Brightening Cream, the SkinMedica TNS Essential Serum, and the Alastin Skin Nectar stimulate the skin to form such collagen. These are outstanding products that are recommended before and after treatments. While light chemical peels and laser skin tightening treatments can improve acne scars, usually deeper treatments are required. These are usually fractional laser treatments such as the Pro Fractional Therapy. The Pro Fractional Therapy creates microscopic holes in the skin, stimulating the skin to form healthy collagen, leading to a smooth skin surface. Recovery after the treatment consists on redness for up to ten days and multiple treatments are required. Dr. Alex recommends a series of at least five treatments every four to eight weeks. It takes time to see the improvement because it creates new collagen and collagen does not form overnight. The full results are seen six to twelve months after the final treatment.

The active ingredients in the sunscreen are physical blockers: zinc oxide and titanium dioxide. Zinc and titanium have quickly become the most popular ingredients in sunscreens for a number of reasons. First, these are physical blockers, which means they reflect the light. Chemical sunblocks absorb the light; they need to be applied 30 minutes prior to sun exposure for full effect. With zinc and titanium as in the A E Skin Sunscreen, you can put on your sunscreen and go out right away. Second, zinc and titanium optimally protect your skin from both UVA and UVB rays, which damage the skin in different ways. UVA rays are thought of as aging rays, which lead to wrinkles, broken capillaries, and sun spots. UVB rays are what we call burning rays, they lead to sunburns. When sunscreen contains these two components it is called broad spectrum, the best type of protection available.
We all want our skin to look the best it can, which is why we spend so much time, effort, and energy undergoing treatments and using skin care products. After an initial series of treatments periodic maintenance treatments help us protect our investment.
What does the initial series of IPL photofacial treatments do?
An initial series of Forever Young BBL IPL photofacial treatments will correct the damage in the skin (mostly sun damage). The Forever Young BBL IPL photofacial treatment uses bright light to target pigment and blood in the skin. So it will improve a number of signs of aging such as dark spots, age spots, redness, and broken capillaries. The Forever Young BBL IPL photofacial treatment further rejuvenates the skin by stimulating the skin to form new, fresh, and healthy collagen, giving you that youthful appearance. A scientific study from Stanford University has shown that the Forever Young BBL IPL photofacial treatment will actually change the genes in your skin to be more youthful.
What does a maintenance IPL photofacial treatment do?
Once you have undergone an initial series of Forever Young BBL IPL photofacial treatments, your skin has improved in a number of ways as discussed. The damage in your skin from years of sun exposure has been corrected. The maintenance treatment will protect your investment by correcting the little sun damage that has occurred since your initial series. It will also be preventive and protect you from damage in the future.
But I don't see the dark spots coming off
Often times when having an initial series of Forever Young BBL IPL photofacial treatments, you will see dark spots getting darker before they get lighter. Sometimes these spots will flake off. If these are age spots, once they are removed they will likely not come back again. So at a maintenance treatment you will not necessarily see these coming off. This doesn't mean that the treatment is not doing anything. On the contrary, there are microscopic areas of sun damage on the skin which are improving. Since these areas are so tiny you will usually not see dark spots flake off or even get darker. But it is important to have your maintenance treatments in order to protect your investment.
How often do I need a maintenance IPL photofacial treatment?
Dr. Alex recommends an initial series of three to six Forever Young BBL IPL photofacial treatments spaced out every four to eight weeks. After that, Dr. Alex recommends one maintenance treatment every three to six months.
What is the difference between maintenance IPL photofacial treatments and maintenance Botox treatments?
Maintenance is maintenance, regardless of the treatment. Botox helps improve certain wrinkles on the skin and Botox will usually last three to four months. At this time the wrinkles become apparent, letting you know that it is time for a maintenance treatment. With the Forever Young BBL IPL photofacial treatment, this is not necessarily the case. This is because the Forever Young BBL IPL photofacial treatment will correct sun damage, which comes back slowly and in microscopic sizes. So you don't wake up one day and all of a sudden see dark spots everywhere. This makes it difficult for us to know when it is time for a maintenance treatment. Dr. Alex recommends scheduling a maintenance Forever Young BBL IPL photofacial treatment at your last appointment in your initial series. This way you are ensured that your investment is protected.
What skin care products should I use after a Forever Young BBL IPL photofacial treatment?
Skin care products are outstanding complements to any types of treatment. The basic products are sunscreen, facewash, and moisturizer. It is important to find products that work well for you. The most popular combination treatments are the Soft and Smooth Rejuvenation and the Tight and Bright Rejuvenation. The Soft and Smooth Rejuvenation combines a Forever Young BBL IPL photofacial treatment with a Micro Laser Peel, which removes a thin layer of dead skin cells from the surface. This will exfoliate the skin that creates a rough and irregular texture as well as clog the pores, so an improvement will be seen for overall anti-aging as well as for acne. Injectable treatments such as Botox, Juvederm, Restylane, Radiesse, and Voluma are outstanding to address various signs of aging and for facial rejuvenation. For this reason they are some of the most common treatments we perform at our Encino, CA office at A E Skin. But any time you have such a treatment, there are risks such as bruising.
What causes bruising from Botox?
Treatments such as Botox, Juvederm, Restylane, Radiesse, and Voluma (and now Kybella for the double chin) use needles to inject products into the skin. The skin contains a variety of different tissues including small blood vessels. If the needle passes through a blood vessel, the blood can come out of the vein or capillary and leak into the surrounding skin, leading to the appearance of a bruise.
How can I prevent getting a bruise after a Botox treatment?
Since the tiny blood vessels in the skin are microscopic, injectors cannot see them when performing Botox, Juvederm, Restylane, Radiesse, or Voluma treatments. So unfortunately it is impossible to completely prevent bruising after these treatments. There are some things we can do to minimize the risk of bruising. We recommend stopping the use of blood thinners such as aspirin for a few days before and after your treatment. At A E Skin in Encino, CA we use cold air during our Botox treatments which serves two purposes: it increases your comfort and it decreases the risk of bruising. We also provide complimentary ice packs to all patients receiving Botox, Juvederm, Restylane, Radiesse, and Voluma. Applying a cold compress such as an ice pack after these treatments will also decrease the risk of bruising.
How long does a bruise from a Botox injection last?
Bruises from injections with treatments including Botox, Juvederm, Restylane, Radiesse, and Voluma can last up to two weeks, or sometimes longer. This depends on the size of the bruise and the individual. Small bruises will go away faster. Some people recover from bruises more quickly than others.
Can bruises be treated?
Yes, bruises can be treated with lasers. Dr. Alex has done thousands of laser treatments using lasers for bruising, dark spots, redness, leg veins, skin tightening, toenail fungus, and more. The treatment of a bruise is simple and takes less than one minute. The laser is applied to the bruise and a few pulses are all that is required. The light from the laser is absorbed by the blood, destroying the bruise. The treatment is comfortable and patients tolerate it very well. Usually the bruise is significantly better if not completely resolved in 24 hours.
