Botox before and after for TMJ painBotox is commonly used to address wrinkles in the face, but it can also be used to address TMJ pain and to contour the face.

What is Botox and what does Botox do?

Botox is a purified protein most commonly used to address wrinkles associated with facial expression. These include the horizontal lines seen on the upper forehead when we raise our brows, the vertical lines seen between the brows when we frown, and the crow’s feet seen around the eyes when we smile or squint.

What is TMJ pain?

TMJ is short for temporomandibular joint. This is the joint that we use on our face when we move the jaw to eat or speak. TMJ pain occurs when there is excessive use of the joint or excessive pressure on the joint. This occurs frequently in people who grind or clench their teeth. The joint can become inflamed, swollen, and painful. If severe it can even lead to migraine headaches.

How does Botox treat TMJ pain?

To treat TMJ pain Botox is placed in the masseter muscle in the lower face. This is the muscle on the sides of the jaw that we use to bite. In people with TMJ disorders this muscle sometimes gets strong and large and sometimes a bulge is seen there. When injected into this muscle, Botox will relax the muscle. As a result the strength in which a person grinds or clenches is reduced significantly. This decreases the stress on the TMJ joint and will lead to decreased levels of stress on the joint, relieving the inflammation, pain, and swelling.

How does Botox contour the face?

When Botox is placed into the masseter, the muscle itself gets smaller, which decreases the bulge on the lower face on the sides of the jaw. This bulge can give a masculine square jaw appearance. When the muscle is reduced in size, decreasing the width of the lower face, providing a feminine heart shape to the lower face.

How long does it last?

A Botox treatment for TMJ pain or to contour the lower face typically uses higher doses than what is used in the upper face. So most patients enjoy their results for longer than the typical three to four months, sometimes up to six to nine months. At that time a maintenance treatment is recommended.

What are the side effects of Botox?

When the masseter muscle is treated with Botox, the muscle becomes relaxed and it is difficult to use the muscle, which prevents you from grinding your teeth. At the same time it does make it more difficult to bite or chew. This takes some time to adjust to. With any injection there is a risk for pain, bleeding, bruising, and infection. At A E Skin we take precautions to minimize such adverse effects, For example, we thoroughly clean the area with alcohol prior to treatment and use a painless technique to maximize your comfort. We provide complimentary laser bruise treatments as early as the next day; this makes the bruise disappear more quickly. There is also the possibility that Botox spreads to nearby muscles, which can lead to a temporary asymmetric smile.

What should I do after a Botox treatment?

After a Botox treatment please avoid heat exposure, alcohol consumption, and strenuous exercise for 24 hours and do not lie down flat for four hours.

Are you dealing with a double chin that makes you feel downright embarrassed? What is Kybella?

Instead of stressing over needing surgery, patients can now elect to have a non-surgical injection that uses deoxycholic acid to destroy the fat cells within submental fat. One of the great things about Kybella is that it uses a naturally occurring chemical in your body that aids in fat absorption to help kill fat cells in the chin.

Will I need multiple treatment sessions or just one?

Most patients who get Kybella will need more than one treatment. Sometimes up to six treatments will be necessary. Factors such as how much chin fat is being treated and how the patient responds to treatment will determine how many sessions you will actually need to achieve the results that you want. It’s important that before we even start treatment that you talk to us honestly about your treatment goals so we can determine if Kybella is right for you or if another treatment would fit your goals better.

When will I notice results?

This will depend on how many treatment sessions our Encino skin doctor recommends. If you are getting multiple treatment sessions they are usually spaced a few weeks apart (anywhere from 6-8 weeks).

Lip enhancement before and afterWe are excited to announce that Juvederm Volbella is now available for lip enhancement at A E Skin.

What is Volbella?

Volbella is the newest member of the Juvederm family, which includes Juvederm and Voluma. The Juvederm family is a line of dermal fillers used to restore volume to the face. As we age we naturally lose volume in various areas. At the same time there are structural changes in the bones and we lose collagen in the skin. As a result we see signs of aging such as sagging skin, volume loss in the apples of the cheeks, parentheses lines, jowls, and smaller lips. The Juvederm family of products is designed to counteract these changes and provide a youthful, healthy, and rested appearance.

What does Volbella treat?

Volbella is specifically designed for the lips and the areas around the lips. In the lips it provides volume and can shape the lips, enhancing the borders and improving the definition. It will also smoothen out the lines that form on the lips over time. In the areas directly around the lips Volbella can fill in fine lines such as the smokers lines which are frequently seen as we age in smokers or non-smokers.

Although the pivotal studies were performed on the lips and the product is approved for volumization in the lips, Volbella can also be used off label for other areas such as the tear troughs, or the under eye hollows. Previously other products such as Restylane have been used. But now with Volbella, the results last about twice as long.

Volbella Lip Injection Demonstration

When will I see results from Volbella?

The great thing about fillers such as Volbella, Voluma, Juvederm, and Restylane is that results are seen right away. Any time you receive an injection there is swelling in the area, which can provide a desired appearance. The swelling usually lasts up to two weeks, but could last longer. At this point final results are seen. We have patients return two weeks after their treatment to assess the results and determine if further correction is desired.

How long does Volbella last?

Volbella uses Vycross technology, just like Volbella. This is a new technology where there is a higher level of cross-linking of the hyaluronic acid. The effect is that you will enjoy the results for a longer time. Most fillers in the lips last about six months, but Volbella has been demonstrated to last one year.

What is Volbella made of?

The active ingredient in Volbella is hyaluronic acid, a naturally occurring substance found in the skin that binds 1,000 times its weight in water. This is the same active ingredient in many other fillers such as Juvederm, Restylane, Voluma, and Belotero.

What are the side effects of Volbella?

Common side effects with any injection include pain, bleeding, bruising, redness, swelling, lumps and bumps, itching, discoloration, and infection. Most of these adverse effects are mild to moderate and resolve within 30 days. The most rare side effects occur when the product is injected in or around and artery, which blocks the blood supply to certain areas. This may lead to necrosis, scars, visual abnormalities, and neurological changes such as blindness or stroke. For Dr. Alex the most important feature of Volbella and other hyaluronic acid fillers such as Juvederm, Restylane, and Voluma is that they are reversible. So in the event that a serious adverse event occurs, the product can be dissolved. If done so quickly the effects can be reversed.

When was Volbella released?

Volbella has been used in Europe since 2011 and is now available in over 70 countries worldwide. The pivotal studies for the FDA have now been completed and it is available as of October 17, 2016. Dr. Alex completed the required training course and is certified to inject Volbella.

Is Volbella safe?

The safety and efficacy of Volbella was demonstrated in the pivotal study in which 168 patients were treated with Volbella. The most common side effects resolved within 30 days and include redness, tenderness, swelling, bruising, firm lumps or bumps, discoloration, and itching. The majority of patients treated with Volbella were happy with their results at one year.

Dr. Alex and his family with Violet on the Red CarpetDr. Alex attended The Aesthetic Academy in Irvine October 15-16, 2016. The Aesthetic Academy is a smaller and regional version of the annual Aesthetic Show. This conference was another opportunity for Dr. Alex to learn about new techniques, products, and device in aesthetics. The conference was held at the Irvine Marriott hotel, which happens to be a couple of blocks from the Allergan corporate office (makers of BotoxJuvedermVoluma, LatisseKybella, and now Volbella). Dr. Alex had a chance to visit the office the day prior to the conference. This year A E Skin was honored to be nominated for Best Aesthetic Practice.  Dr. Alex had the opportunity to participate in the injectable course, a full day training session designed for doctors and nurses to improve their skills. The morning session included lectures on various topics such as preventing Botox complications, using fillers for facial rejuvenation, and new injectables such as Kybella to treat fullness in the neck. Expert speakers from around the country included Dr. Andrew Ordon of the hit television show The Doctors, Dr. Jason Emer, and Dr. Monica Banakdar. The afternoon session allowed Dr. Alex to observe live injections by these experts. Dr. Alex learned new and different techniques to improve outcomes, decrease pain, and minimize side effects. Twin smokers

We all know the risk of smoking on the health including lung damage, heart disease, increased risk of cancer, and others. But many people do not know the effects that smoking cigarettes has on the skin over time. A new study conducted by Dr. Haruko Okada and colleagues at Case Western Reserve University in Cleveland, Ohio published in the November 2013 issue of Plastic and Reconstructive Surgery demonstrates these effects. The study compared photographs of twins, one of which smoked and the other did not, or one of which smoked at least five years longer than the other. The photographs of the smokers demonstrated significantly more signs of facial aging including eyelid laxity, under eye bags, fine lines, deep lines, sagging skin, and discolorations. In the image from the study shown here, the twin on the right smoked 14 years longer than his brother. Although they are the same age, the smoker has more wrinkles and deeper wrinkles and appears older. Like sun exposure, smoking leads to the formation of harmful free radicals in the skin. These harmful free radicals have multiple effects. They damage the collagen in the skin, the protein that holds the skin tight, leading to fine lines, wrinkles, and sagging. It also leads to discolorations which are commonly known as age spots. Sunscreen can block the harmful effects of the sun, but it can not protect the skin from the free radicals generated from smoking. Topical antioxidants such as vitamin C can neutralize these free radicals to prevent aging. But the best way for smokers to prevent aging is to quit smoking. Other elements of a healthy lifestyle include sun protection, eating a healthy diet, getting regular exercise, and getting enough sleep. Making healthy lifestyle choices will lead to healthy and youthful appearing skin

Chemical Peel Aftercare Instructions

For best results, carefully follow these aftercare instructions: Please avoid heat exposure, alcohol consumption, and strenuous exercise for 24 hours after the treatment. Heat refers to going out in the hot sun, if it were to be hot outdoors, but also any hot environment indoors, such as a hot bath, sauna, or steam room. Also avoid hot or spicy foods or drinks like soup, hot coffee, or hot tea because all of these things will bring blood to the skin and cause problem like redness, blister, and discolorations. Avoid harsh rubbing or scrubbing for one week after the treatment. Do not use products such as washcloths or the Clarisonic brush. Avoid direct water contact from the showerhead when in the shower. Please avoid makeup for several days after the treatment because your skin will be sensitive. You may be red for a few days after the treatment. Redness is normal, don't be alarmed. Feel free to apply cold compresses to your skin to keep you comfortable, if necessary. Sometimes dark discolorations get darker before they get lighter; this is normal. For the week after your treatment, spray a small amount of the Avene thermal water then apply a thin layer of the Elta MD Intense Moisturizer every few hours to improve the healing process. Be sure to apply a broad spectrum sunscreen with SPF of at least 30 every day.

What Happens After a Chemical Peel?

Most people will peel on days three to five after the treatment, but everyone is different. Some people don't peel at all. This is ok, for the goal is not to peel, it is to get results. These people are actually getting exfoliation, but it is microscopic, so you don't see it. If you do get visible peeling, please do not pick at it, otherwise you may have side effects such as redness, blisters, and discolorations. By seven days, the new skin has formed. It is refreshed and rejuvenated, and you will see improvements in color texture and tone.
