Vollure received approval from the FDA on March 20, 2017 for the correction of moderate to severe facial wrinkles and folds in people age 21 and over. These wrinkles include the nasolabial folds, also called laugh lines, smile lines, or parentheses lines, which go from the sides of the nose to the corners of the mouth. The nasolabial folds are common concerns as we age and sometimes continue to the jawline as the marionette lines.
Vollure was first approved in Europe in 2013 under the name Volift and uses Vycross technology. Vycross technology blends different sizes of the active ingredient hyaluronic acid. This allows for greater longevity of the product. Most people enjoy the results of their Vollure treatment for up to 18 months with optimal correction. This is in contrast to only 12 months with Juvederm. In the pivotal studies 96% of patients treated with Vollure reported improvement six months after their treatment and 62% of patients reported improvement 18 months after their treatment. The Vycross technology is now used in three fillers in the Juvederm family: Voluma, Volbella, and Vollure. Voluma is designed to restore lost volume to the upper cheeks, providing an instant lift and Volbella is designed to address fine lines on and around the lips.
The difference between the three products in the Juvederm family that use the Vycross technology, Voluma, Volbella, and Vollure, is the amount of hyaluronic acid in the product. Hyaluronic acid is a naturally occurring sugar found in the body that binds 1,000 times its weight in water. When hyaluronic acid fillers are placed in the skin they draw water into the area to restore volume. Voluma has a high concentration of hyaluronic acid (20 mg/mL). Therefore, Voluma draws in a lot of water and is able to provide the volumization and instant lift. Volbella, on the other hand, has a lower concentration of hyaluronic acid (15 mg/mL). Volbella does not draw as much water into the skin and is ideal for fine lines around the lips. Being a softer filler, Volbella is also being used off-label to treat hollowing under the eyes. Vollure has an intermediate amount of hyaluronic acid (17.5 mg/mL) and is suited for areas such as the nasolabial folds. Vollure can also be used in the lips. In the lips Vollure is used like Juvederm to provide volume whereas Volbella is used for fine lines and defining the borders.
Treating wrinkles with Vollure is fast and safe and leaves no scars or other traces on the face. Most side effects are mild or moderate in nature, and their duration is short lasting (seven days or less). The most common side effects with Vollure include, but are not limited to, temporary injections site reactions such as redness, pain/tenderness, firmness, swelling, lumps/bumps, bruising, itching, and discoloration. As with all skin injection procedures there is a risk of infection. People with a history of cold sores may experience a recurrence after the Vollure treatment, although this can be minimized by the use of antiviral medicines. The area around the eyes is a common concern because changes occur in this area as we age that make us look old and tired. There are a number of options that work well to enhance the appearance of the eyes and provide a more healthy and rested look.
Changes in the Eye Area
As we age there are a number of changes that occur in the eye area. The brows descend and can feel heavy. The lashes thin out. Wrinkles appear around the eyes. Dark circles and hollowing are seen under the eyes. And bags can occur under the eyes as well.
Restylane is a dermal filler that works well to address hollowing under the eyes. As we age the fat pad we have under the eyes at birth moves downward into the upper cheek. As the volume is lost a hollow can appear under the eyes, giving a tired appearance. Restylane is a hyaluronic acid product that is placed in this area to restore the volume and give the rested and refreshed appearance. Hyaluronic acid is a naturally occurring sugar found in the skin that binds 1,000 times its weight in water. It will bring water into the area and provide the youthful fullness to the under-eye area. The results of a Restylane treatment are seen right away. There is usually a good amount of swelling after a Restylane treatment and the final results are seen approximately two weeks later once the swelling has subsided. Most people enjoy the results of their Restylane treatment for six to 12 months.
The A E Skin Enlightening Eye Cream is one of our most popular products for enhancement of the eye area. It is a product that Dr. Alex recommends to all patients with concerns in the area because it improves the appearance of the eye area and enhances the effects of the treatments described below. The A E Skin Enlightening Eye Cream delivers rich hydration to the delicate skin around the eyes. It is rich but not too heavy and can be applied to the skin above, below, and around the eyes twice daily. It works well for fine lines, dark circles, and puffiness around the eyes. It contains the perfect level of retinol, ceramide, and peptides to stimulate collagen formation, fighting wrinkles, loose skin, and under-eye bags. Hyaluronic acid draws water into the skin, hydrating it and providing volume. Niacinamide (vitamin B3) calms the skin and brightens discoloration. calms the skin and brightens discoloration. Caffeine, arnica, and vitamin K reduce the appearance of dark circles and puffiness. Green tea extract serves as a powerful antioxidant to protect the skin.
Botox is a purified protein used to address wrinkles associated with aging. Botox is commonly used to address wrinkles in the eye area between the brows and around the eyes. The frown lines are the vertical lines seen between the brows when we smile and the crow’s feet are the lines seen around the eyes when we smile or squint. Botox will relax the muscles we use to make these facial expressions and smoothen out the overlying wrinkles. Dr. Alex uses a painless Botox injection technique to maximize your comfort. The results of a Botox treatment are seen by two weeks and most people enjoy their results for three to four months.
Latisse is a product used to enhance the appearance of the lashes. Latisse comes as an ophthalmic solution. Once drop is placed on the tip of the applicator that comes in the kit and the Latisse applicator is drawn along the area where the upper lash meets the skin starting from the corner of the eye closer to the nose to the corner of the eye closer to the ear. Latisse can also be applied on the eyebrows to promote hair growth there as well. This is repeated with the other lash and brow every night. Initial results with Latisse can be seen in as little as four weeks. Full results are seen at 12 to 16 and you will continue to enjoy the results as long as you keep using the product.

Dr. Alex gave a presentation discussing the various aspects of the liquid facelift and what works for each person. The liquid facelift is a treatment customized for each person which uses liquid products to provide a facelift result without surgery. Products included in the liquid facelift include Botox, various dermal fillers, and Kybella. Botox is a purified protein used to address wrinkles associated with facial expression. The most common areas treated with Botox are the horizontal lines seen on the forehead when we raise the brows, the vertical lines seen between the brows when we frown, and the crow’s feet seen around the eyes when we smile or squint. Dermal fillers such as Juvederm, Restylane, Voluma, and Volbella restore lost volume to the face. Juvederm works well to address the nasolabial folds, or laugh lines. These are wrinkles that go from the sides of the nose to the corners of the mouth; sometimes they extend as marionette lines to the jawline. Dr. Alex uses Restylane to fill in hollowing under the eyes. Voluma is used to restore volume in the mid-face. It gives the apple of the cheek that is lost over time and pulls the lower face upward, improving the nasolabial folds, marionette lines and jowls. Volbella is the newest filler and is used for fine lines on the lips or around the mouth. Kybella is the first and thus-far only FDA-approved, non-surgical, injectable treatment for the double chin. When injected into the skin Kybella will dissolve the fat and it is naturally eliminated by your body slowly over time. A series of two to four Kybella treatments is recommended over several months.
We were excited to present our newest laser: the fractional CO2 by Deka. Fractional CO2 laser resurfacing can address multiple aspects of aging in a single treatment including fine lines, wrinkles, loose skin, dark spots, acne scars, surgical scars, and stretch marks. Fractional CO2 laser resurfacing works by creating thousands of microscopic holes in the skin and generating heat. This stimulates your body to produce its own natural, new, fresh, and healthy collagen, thereby tightening the skin and helping with wrinkles, looseness, scars, and stretch marks. Fractional CO2 laser resurfacing also allows for discolorations to come out of the skin. As this is an aggressive treatment, most people have about one week of recovery time (redness and swelling). However, we now have a lighter version of the fractional CO2 laser resurfacing with a shorter recovery time of three to four days which allows for healing to occur over a weekend.
We also featured a new product by Alastin called the Restorative Complex. Alastin is a skin care line originally designed for pre- and post-procedure to enhance results and reduce recovery time. The proprietary Tri-Hex peptide technology in the Alastin Skin Nectar stimulates the formation of collagen and elastin, proteins that keep the skin firm and tight, and clears the deeper layer of the skin, allowing for faster healing after a treatment. The Alastin Skin Nectar is specifically designed to be used before and after treatments. The new Alastin Restorative Complex is designed for everyday use and includes other ingredients to volumize the skin, making it an ideal complement to fillers such as Juvederm, Restylane, Voluma, and Volbella.
Dr. Alex’s presentation was followed by live demonstrations of Botox, Volbella, and fractional CO2 laser resurfacing. Patients had the opportunity to observe these treatments. Botox was used to address wrinkles on the forehead and to lift the brows. Volbella was used to improve fine lines around the mouth. And the fractional CO2 laser was used to rejuvenate the face.
