What is rosacea?
Rosacea is a condition characterized by the appearance of redness on the face. This usually occurs in the central face on the nose and cheeks, but for some people it can spread to the entire face and even down to the neck. Sometimes you see generalized redness on the skin and in some people, you can see broken capillaries, particularly on the nose and cheeks. In some cases, bumps appear, making it look like acne; and sometimes rosacea is confused with acne. People with rosacea tend to have sensitive skin. With rosacea there are triggers that lead to the redness. Everyone is different, but common triggers include sun exposure, heat, cold, eating spicy food, drinking alcohol, and emotional stress.
How is rosacea treated?
Rosacea is a chronic condition that is controlled, not cured. The first thing to do is to identify the triggers for the individual and avoid them. Sunscreen is important for all patients with rosacea because the radiation from the sun can make everything worse, even if it is not a trigger. Dr. Alex recommends a sunscreen with SPF at least 30 every day. Rosacea patients love the Elta MD UV Clear sunscreen because it contains an ingredient called niacinamide. This is a vitamin B derivative which helps to reduce redness. Treatment for rosacea involves targeting the blood vessels in the skin. This can be done with lasers. We will provide an aftercare kit including a special facial cleanser, the Avene Thermal spray water, and an intense moisturizer with specific instructions on how to use them. Apply sunscreen daily with a minimum SPF of 30 with broad spectrum (UVA and UVB) protection. Treatments for the neck
Common concerns in the neck area include loose skin, fullness or a double chin, and loss of jawline definition. Over time and with sun exposure, collagen in the skin on the neck and on other areas is degraded. This leads to the appearance of loose skin and wrinkles. One of the most popular treatments is laser skin tightening. This is a no down time treatment that takes about 15 minutes. A series of six to eight weekly laser skin tightening treatments is recommended followed by one maintenance laser skin tightening treatment every three months thereafter. The double chin, also known as submental fullness, is very common and caused by age, weight gain, and genetics. The active ingredient in Kybella is a salt used to digest the dietary fat. When injected into this area, Kybella will dissolve the fat and it is naturally eliminated by the body slowly over time. What is an IPL photofacial treatment?
IPL stands for intense pulsed light. It is also called a photofacial, or IPL photofacial. The treatment works by using bright light to target and destroy unwanted pigment and blood in the skin. Sometimes darks spots get darker before they get lighter and they may or may not flake off the skin surface. There may be redness on the skin for a day or two after an IPL photofacial treatment. Otherwise, there is no recovery time after an IPL photofacial treatment; most people resume their normal activities right away. What is striping?
Striping is a possible complication after an IPL photofacial characterized by the appearance of either dark or light stripes on the skin. This can occur as a result of an IPL photofacial treatment. During the IPL photofacial treatment, the tip on the device is placed on the skin to pulse the light. The tip is usually rectangular or square in shape. After a pulse, the tip is moved to the adjacent portion of the skin which has not yet been treated. If there is space between these areas, there will be certain areas of untreated skin. In such cases, the unwanted pigment may be cleared in the treated areas, but not in these untreated areas, making it look like stripes of untreated dark skin between usually larger striped of treated even colored skin. This is a result of not having enough overlap between pulses. On the other hand, if there is overlap from one pulse to the next adjacent pulse, the areas of overlap are effectively treated twice. If the settings are aggressive burns and discoloration can be seen on these thin areas of overlap, also creating striping.

What are the risks of IPL for people with dark skin?
People with dark skin have a tendency to get dark easily after any type of insult to the skin such as sun exposure, acne breakouts, cuts, burns, and laser treatments. This is called post-inflammatory hyperpigmentation (PIH), which means darkening of the skin after inflammation.
How can I prevent PIH?
In order to prevent PIH Dr. Alex recommends using a good skin care regimen such as the A E Skin line of products: Brightening Cream, Exfoliating Serum, Triple Antioxidant Serum, and Hydrating Serum. These products contain a number of ingredients which prevent PIH including hydroquinone, kojic acid, tretinoin, salicylic acid, vitamin C, and vitamin E. Using these products for one to two months prior to a laser treatment significantly decreases the likelihood of PIH.
What does IPL do?
IPL uses bright light to rejuvenate the skin. The two primary targets of IPL are melanin (which gives skin the pigment) and hemoglobin (which is in the blood). This allows for treatment of dark spots, age spots, uneven tone, redness, and broken capillaries. The same melanin that is in discolorations is also in the skin, so the laser targets both of these. We do not want to target the natural pigment in the skin, only the darker spots. And since those with darker skin types have more melanin in the skin, adjustments need to be made.
How is IPL safe in darker skin types?
The Forever Young BBL IPL photofacial device by Sciton is very versatile and allows Dr. Alex to adjust a number of settings and parameters in order to treat darker skin types safely and effectively. The first setting that Dr. Alex adjusts is the wavelength (or color) of light. Lower wavelengths (blue and green) target pigment more strongly than higher wavelengths (orange and red). For people with darker skin types Dr. Alex uses higher wavelengths for safety. This is easily adjusted with a variety of filters available. The next setting that is adjusted is the fluence (or energy per unit area). Lower fluences are used for darker skin types. The pulse width is the length of time the laser is on. The shorter the pulse width the more aggressive the treatment. Longer pulse widths are used for darker skin types. The temperature of the laser tip can also be adjusted. Lower temperatures allow for safer treatments in darker skin types.

Since these areas are so tiny you will usually not see dark spots flake off or even get darker. But it is important to have your maintenance treatments in order to protect your investment.

What is the difference between maintenance IPL photofacial treatments and maintenance Botox treatments?
Maintenance is maintenance, regardless of the treatment. Botox helps improve certain wrinkles on the skin and Botox will usually last three to four months. At this time the wrinkles become apparent, letting you know that it is time for a maintenance treatment. With the Forever Young BBL IPL photofacial treatment, this is not necessarily the case. With fillers in the hands the results are seen right away. There is an initial amount of swelling that goes away after about two weeks, which is when the final results are seen. Sometimes you see a coffee ground appearance and they flake off. Other times the pigment is absorbed by the skin. This can take up to two weeks to clear.
Does the hand rejuvenation treatment hurt?
Dr. Alex uses a painless technique when performing hand fillers. An anesthetic is injected into the back of the hand to keep you comfortable and he uses a blunt cannula instead of a sharp needle. How long do the hand rejuvenation results last?
Most people enjoy the results of fillers such as Juvéderm in the hands for six to twelve months. The more active you are with your hands the more frequently you will require a maintenance treatment. For the dark spots, most people enjoy results with just a single Forever Young BBL IPL photofacial treatment treatment, but some people require a series of up to six treatments. After that maintenance treatments are done once every three to six months.
