On Friday May 18, 2018 Dr. Alex attended the Allergan Medical Illumination Tour at the Westin hotel in Pasadena, CA. This is an event held in numerous cities nation wide to teach doctors about the newest technology and techniques in the use of dermal fillers for facial rejuvenation. The speakers with plastic surgeon Dr. Arthur Swift of Montreal, Canada and oculoplastic surgeon Dr. Steve Fagien of Boca Raton, FL.
What are dermal fillers?
Dermal fillers are injectable products used to restore volume in the face. Botox and dermal fillers are two of the most commonly used products for facial rejuvenation. Dermal fillers can be used to fill lines and folds such as the laugh lines (or nasolabial folds), improve hollowing under the eyes, restore the youthful apple of the cheeks, straighten a nose with a hump, enhance a weak chin, rejuvenate the jawline, and restore volume in the temples.
The products covered in the Allergan Medical Illumination tour were the fillers in the Juvederm collection. These include Juvederm Ultra and Juvederm Ultra Plus, the first ones available, as well as the newer products Voluma, Vollure, and Volbella. All of these products use a sugar found naturally in the skin called hyaluronic acid as the active ingredient. This brings water into the skin to restore volume and fill in wrinkles and folds. The newer products (Voluma, Vollure, and Volbella) use a new technology which allows you to enjoy your results longer than Juvederm Ultra and Juvederm Ultra Plus. These products are also softer, which allows them to move when you make facial expressions, providing a natural appearance.
The theme of the talk was light and shadows. These are two important things to understand when thinking about beauty. Makeup artists think about light and shadow all the time. The reflection of the light to highlight certain areas of the face is very important for beauty. At the same time, shadows formed on the skin provide a three-dimensional appearance of a contoured face. Using these ideas, the products are placed strategically in certain areas to maximize the effects of light and shadows. This allows for outstanding results without the requirement for a large amount of product. For example, certain points on the cheek should have maximum projection and more product is place there. Additionally, understanding anatomy allows injectors to place the products at the appropriate depth for safety as well as ideal results.

This treatment uses multiple liquid products such as Botox, Voluma, Vollure, Juvederm Ultra, and Volbella to improve wrinkles and restore lost volume to the face. As compared to a traditional surgical facelift, the liquid facelift does not involve anesthesia, surgery, or recovery. The liquid facelift treatment itself is customized for each person.
The patient whose picture is shown here saw significant improvement in her skin with the liquid facelift. This was complemented by using the A E Skin line of products. In this case 54 units of Botox, two syringes of Voluma, three syringes of Vollure, and two syringes of Volbella were used. This treatment provides over 10 years of youth. Botox is used in the non-surgical facelift to address wrinkles associated with facial expression. Usually this is in the upper face. The most common areas treated with Botox are the horizontal lines seen on the upped forehead when a person raises the eyebrows, the vertical lines seen between the eyebrows when a person frowns, and the crow’s feet seen around the eyes when a person smiles or squints. Typically, 50-60 units of Botox are used in this area. Botox can also be used in the lower face to relax muscles that pull the face downward. This will provide a further lift.
Fillers are used to restore lost volume in the skin. The Juvederm family of products is the most popular group of fillers in the United States for a number of reasons. First, the active ingredient in the Juvederm family is hyaluronic acid, a naturally occurring sugar found in the skin and elsewhere in the body. It binds 1,000 times its weight in water and draws water into the skin to fill the wrinkles and folds. It has a high safety margin because it can be dissolved in the case of undesired results or a rare adverse event. The Juvederm collection of fillers includes the Vycross family: Voluma, Vollure, and Volbella. These products use a new technology that allows patients to enjoy their results longer. It also provides very natural results.
Voluma is usually used in the upper cheeks to restore the apple of the cheeks. This provides a youthful appearance to the face and pulls up on the lower face. It can help improve the appearance of jowls and provide a more defined jawline. Voluma is a firm product and provides a noticeable but natural lift that is seen immediately. Unlike a traditional surgical facelift where only the skin is tightened, when using products such as Voluma, volume is restored to the face. Most people start with one syringe of Voluma per side, a conservative amount. The results of the Voluma treatment are assessed approximately two weeks later and more product can be added at that time if further enhancement is desired.
Vollure is the second filler used in the liquid facelift. Vollure is slightly softer than Voluma and is commonly used in the nasolabial folds (the lines that go from the sides of the nose to the corners of the mouth) and the marionette lines (the lines that go from the corners of the mouth to the jawline). Most people start with two to three syringes of Vollure in this area, a conservative amount. The results of a Vollure treatment are assessed approximately two weeks later and more product can be added at that time if further enhancement is desired.
Juvederm Ultra is the original Juvederm product that uses the previous Hylacross technology. Similar to Vollure, Juvederm Ultra is used in the nasolabial folds and marionette lines. The difference is that Juvederm Ultra contains a higher level of hyaluronic acid, which means it brings even more water into the skin. Juvederm Ultra is best suited for those who have deeper folds in this area. Most people start with two syringes (one per side) of Juvederm Ultra in this area, a conservative amount. The results of a Juvederm Ultra treatment are assessed approximately two weeks later and more Juvederm Ultra can be added at that time if further enhancement is desired.
Volbella is the softest filler available in the Juvederm collection of fillers. It is ideal to address fines lines on or around the lips as well as shaping the lips. Volbella can also be used to restore lost volume to the tear trough area. This is the area directly below the eyes where hollowing is commonly seen as people mature. Most people start with two syringes of Volbella in either of these areas, a conservative amount. The results of a Volbella treatment are assessed approximately two weeks later and more product can be added at that time if further enhancement is desired.
The liquid facelift is an ideal treatment for anyone who desires a more youthful appearance to make them look as good as they feel. When these treatments are combined, patients really love their results.
Changes that occur around the eyes are common. For example, hollowing under the eyes occurs naturally over time and can be associated with a number of things such as age, weight loss, lack of sleep, and genetics. Depending on the person, a number of treatment options are available.
The first piece of advice for anyone with concerns in the area around the eyes is to use a good sunscreen and a good eye cream every day. Look for a sunscreen with SPF of at least 30 with broad spectrum protection. Loose Skin Around the Eyes
If the skin around the eyes is loose and has fine lines and wrinkles, treatments should address that. These fine lines and wrinkles occur due to lack of collagen. Collagen is the protein in the skin that keeps the skin firm and tight. As we mature we lose collagen and as a result fine lines are seen over time. Treatments such as the fractional CO2 laser resurfacing can provide outstanding results for such concerns. There may be a week or more of recovery time – redness and swelling. But the results can be outstanding and last for years. This treatment sometimes works for bags under the eyes because it tightens the skin. It will also help with pigment in the area.
Hollowing Under the Eyes
Another common concern is hollowing under the eyes, which is very common. This is due to volume loss that occurs naturally over time. At birth we have a fat pad directly under the eye. As we age, the fat pad gets smaller and the bony structure of the face changes such that the fat pad also descends. This volume loss is seen as hollowing under the eyes. Fillers such as Volbella can be placed to restore lost volume under the eyes. This is a quick treatment and most people enjoy their results for six to 12 months.
Crow’s feet are the lines seen around the eyes (between the eyes and ears) when people smile or squint. These are caused by muscles contracting and can be treated with Botox to relax the muscle. Botox is one of the most popular cosmetic treatments in the world. It is very fast, safe, and effective. Full results are seen by two weeks, but most people start to notice improvement in the first three to seven days. Most people enjoy their results for three to four months, at which point a maintenance treatment is recommended.
The eyelashes are also a common concern. Over time we lose the thickness and fullness of the hairs in this area. Latisse is a prescription-strength product that is applied nightly and leads to darker, fuller lashes. Initial results are seen in about four weeks and full results are seen after three to four months of nightly use. As long as you continue using it, you will continue to enjoy the results. Many people also apply it on the brows to thicken the brows.
Dermal fillers such as Juvederm and Vollure are some of the most common products we use for facial enhancement. New products become available regularly. Here we discuss the difference between Juvederm and Vollure, as well as other dermal filler products.
What are dermal fillers?
Dermal fillers such as Juvederm, Voluma, Vollure, and Volbella are products that are used to restore lost volume in the skin. They have been used for many years for the smile lines, also called the laugh lines, parentheses lines, or nasolabial folds, the wrinkles that go from the sides of the nose to the corners of the mouth. Sometimes these wrinkles continue all the way down to the jawline, what are called the marionette lines. Fillers are also used to restore the apple of the cheeks, fill in hollowing in the temples, provide volume to the lips, and enhance the chin. The can also be used off the face, most commonly to restore lost volume to the backs of the hands.
What are dermal fillers made of?
Dermal fillers can be made of many different substances. The most popular are those with hyaluronic acid as the active ingredient. These include Juvederm, Voluma, Vollure, Volbella, the Restylane family of products, Belotero, and Revanesse. Hyaluronic acid is a naturally occurring sugar found in the skin that binds 1,000 times its weight in water, allowing it to draw water into the skin to fill in wrinkles. The great things about hyaluronic acid is that is can be dissolved if the results are not as desired or if there is an adverse effect. No other product has this safety feature. Radiesse is another dermal filler whose active ingredient is calcium hydroxyappetite, which works well to restore bony structures such as the cheeks. Sculptra is made of poly-L-lactic acid. This does not volumize by itself; rather it stimulates the skin to form collagen. Bellafill is another dermal filler; it’s active ingredient is poly-methylmethacrlate. Bellafill lasts many years. There are also many other products available overseas but not in the United States. Unlike the hyaluronic acid products, Radiesse, Sculptra, and Bellafill are not reversible.
Voluma, Vollure, and Volbella have some distinct advantages over other fillers. First, they are all made of hyaluronic acid, meaning they are reversible, providing a high degree of safety. Second, unlike some of the other hyaluronic acid fillers, they contain lidocaine, which numbs the skin. Third, they provide patients with Brilliant Distinctions points. The Brilliant Distinctions program is a rewards program developed by Allergan, makes of Botox, Juvederm, Voluma, Vollure, Volbella, Kybella, and Latisse, where patients receive points for each treatment which can be used toward discounts at subsequent treatments. The most unique feature of Voluma, Vollure, and Volbella is that they use a new technology called Vycross, in which the hyaluronic acid has special cross-linking. This is a fancy way of saying that patients enjoy their results longer than other products.
Juvederm was the original dermal filler produced by Allergan, makes of Botox, Juvederm, Voluma, Vollure, Volbella, Kybella, and Latisse. Juvederm is made using an older technology called Hylacross. The new fillers Voluma, Vollure, and Volbella use the Vycross technology, which allows patients to enjoy their results for longer.
We know that Voluma, Vollure, and Volbella all use the Vycross technology which allows patients to enjoy their results for longer. But what are the specific differences between these three products. All of these products are identical except for the amount of hyaluronic acid they contain. Voluma contains the highest concentration of hyaluronic acid in this group (20 mg/mL). Volbella contains the lowest amount of hyaluronic acid in this group (15 mg/mL). And Voluma contains an intermediate amount of hyaluronic acid (17.5 mg/mL). What this means is that they are used for different concerns. Since Voluma has the highest concentration of hyaluronic acid, it provides more lift and is ideal for restoring the apple of the cheeks, filling in temporal hollows, and enhancing a weak chin. Volbella has the lowest amount of hyaluronic acid and does not provide as much volume or puffiness, making it ideal for fine enhancement of the lips such as filling fines lines on or around the lips and defining the lips, as well as for placement in the tear troughs, the hollows which can appear under the eyes. Vollure is ideal for the smile lines and for volumizing the lips. The video shown here demonstrates a liquid facelift, where all three of these products are used in different areas in combination with Botox.

Although the majority of those undergoing cosmetic treatments are women, more and more men are interested in rejuvenating and enhancing their appearance. Here we discuss the many opportunities for men to improve their appearance in order to look and feel their best. Just like women, men enjoy a natural youthful appearance that improves their confidence and self-esteem.
Skin care is essential for everyone, men and women alike. There are many options available, and a skin care regimen should be customized for each person. For example, some people have more oily skin while others have more dry skin and still others have combination skin. Most men prefer a simple regimen. In such cases, the basic elements include daily sunscreen use along with twice daily cleansing and moisturizing. One of the most popular products for men is the Elta MD UV Clear sunscreen. Botox is one of our most popular treatments for both men and women. Instead, they say that the person looks rested and refreshed. Using appropriate dosing and targeting specific muscles a natural look can be achieved with a Botox treatment. In general men require higher doses of Botox than women. This is because men in general have stronger and larger muscles than women, both on the face and body. Also, the placement of the product on the forehead is usually different for men than women. This is because men usually have more flat brows whereas women prefer arched brows.

Dermal fillers restore volume that is lost over time. Such volume loss occurs in different areas on the face and occurs in both men and women. However, it is important to understand the differences between the facial structures of men and women. Specifically, women prefer broad and high cheek bones and a narrow lower face. Men, on the other hand, usually have more volume in the lower face. Laser treatments can address a variety of concerns on the skin including uneven color, redness, and fine lines and wrinkles. Such changes occur naturally over time and sun damage contributes to it. Depending on the concern, various lasers can be used. The original members of this family are Juvederm Ultra and Juvederm Ultra Plus. These use a proprietary cross-linking technology called Hyalacross. This is a fancy way of explaining the chemistry in the product. The three newest members of the family include Voluma, Vollure, and Volbella. These use a newer technology called Vycross, which allows them to last longer. Now Dr. Alex recommends these products as they provide longer lasting results and the results are very natural. The difference between Voluma, Vollure, and Volbella is the thickness of the products and what they are used for. Voluma has the highest level of hyaluronic acid, making it the firmest product in this group. Voluma is ideal for restoring lost volume in the upper cheeks and mid-face. Often times an immediate lift can be seen. Volbella, on the other hand, has the lowest amount of hyaluronic acid, making it the softest product in this family. Vollure has an intermediate level of hyaluronic acid. Vollure works well for the nasolabial folds or laugh lines, which go from the sides of the nose to the corners of the mouth, as well as the marionette lines, which are sometimes seen as the continuation of the nasolabial fold to the jawline. Vollure is also great for those wanting more fullness to the lips.
What are the results when the Juvederm products are combined?
When these products are used in combination beautiful results can be achieved. Typically, the results are subtle and natural, providing a rested or refreshed appearance. Dr. Alex recommends starting with a conservative amount of product, as more can always be added later. Conservative amounts usually start at two syringes, or one per side. The scar under her lip on the right side of the photograph was not treated. This patient enjoyed a natural result as seen by the lift in the mid face, improvement of the nasolabial folds, and the appearance of a heart-shaped lower face.

In the last few years Allergan has release Voluma, Volbella, and Vollure. These new products are made using a novel technology called Vycross technology.
What is Vycross technology?
Vycross technology is what is used to make the newest members of the Juvederm family, Voluma, Volbella, and Vollure. The previous technology used to make Juvederm Ultra and Juvederm Ultra Plus was Hyalacross technology. With Vycross, different sizes of hyaluronic acid are used and the amount of cross linking between the molecules is higher. This allows the products to last longer.
Voluma, Volbella, and Vollure are use the Vycross technology. The difference is the amount of hyaluronic acid they contain and consequently what they are used for. Voluma has the highest level of hyaluronic acid, making it a thick and robust product used to restore volume in the upper cheek and lift the lower face. Volbella, on the other hand, has the lowest level of hyaluronic acid. This makes it a softer product that is used for fine lines such as those around the lips. Vollure, the newest member of the Juvederm family, contains an intermediate amount of hyaluronic acid. Vollure is used to fill wrinkles such as the nasolabial fold and marionette lines. It can also be used for radial cheek folds which are parallel to the nasolabial folds seen in some people when they smile. Most people enjoy the results of their Voluma, Vollure, and Volbella treatments for two years, 18 months, and 12 months, respectively, with optimal correction. Depending on the individual, multiple products can be combined to provide a youthful and rested appearance.
While cosmetic treatments are very popular with women, more and more men are interested in cosmetic enhancement as well. One of the newer treatments available is Volbella.
What is Volbella?
Volbella is a hyaluronic acid filler in the Juvederm family designed for subtle enhancement of the lips and areas around the lips. Volbella can provide subtle volume or address fine lines on or around the lips. Hyaluronic acid is a naturally occurring sugar found in the skin that binds 1,000 times its weight in water. This draws water into the area where it is injected to restore lost volume and fill in lines and wrinkles. Similar to Voluma and Vollure (the newest filler available in the Juvederm family) Volbella uses Vycross technology. This allows the product to last longer and most people enjoy the results of their Volbella treatment for a full year.
What happens to the lips as we age?
As we age there are structural changes in the bones above and below the lips. There is also volume loss in the lips themselves. This leads to the appearance of smaller lips, loss of the pout, and fine lines. Volbella is used to reverse these changes to provide a natural, yet youthful appearance to the lips.
Why do men need Volbella?
These changes occur in both men and women. Sometimes one lip is smaller than the other lip. In these cases, Volbella can be used to restore the balance. In the photograph seen here a single syringe of Volbella was used in the upper lip only. The amount of product in one syringe on Volbella is approximately one tenth of a teaspoon. This allowed for subtle enhancement of the volume of the upper lip, restoring a natural appearance.

What happens to the face as we age?
There are multiple changes we see in the face as we age. The bones that support the facial structure change in size and position. We lose the fat pads in various portions of the skin that provide us with a youthful look, leading to an older and tired look. We also stop producing as much collagen in the skin. Collagen is the protein that keeps the skin nice, tight, firm, and youthful in appearance. At the same time collagen is broken down by sun exposure, pollutants in the air, and oxidants that get into our body such as from the diet or from smoking. In a youthful face, the volume is in the upper face while the lower face is more thin, giving an inverted pyramid appearance. We also use our facial muscles to produce expressions; over time this can lead to wrinkles formation primarily in the upper face. With age the volume goes down, the face gets deflated and the skin sags. We see volume loss in the apples of the cheeks and under the eyes. Wrinkles appear in the lower face. The jawline definition disappears and jowls form.
What does a liquid facelift do?
A liquid facelift addresses these concerns to restore lost volume, decrease or eliminate the appearance of wrinkles, and provide an overall more youthful and rested appearance. A number of products can be used in this treatment and the specific treatment is tailored to each individual based on their concerns.
Botox is used primarily in the upper face to address wrinkles associated with facial expression. Common areas of treatment with Botox include the horizontal lines seen on the upper forehead when we raise the brows, the vertical lines (frown lines) seen between the brows when we frown, and the crow’s feet seen around the eyes when we smile or squint. Botox will relax the muscles and smoothen out the wrinkles. Botox can also be used in the lower face for a variety of purposes. Botox can be placed around the lips to provide a subtle pout, it can elevate the corners of the mouth, it can improve a bumpy appearance on the chin, it can help with jawline definition (Nefertiti lift), it can reduce the appearance of neck bands, and it can contour the lower face.
Voluma is a dermal filler designed to restore volume to the upper cheeks. With a Voluma treatment you will get back the youthful apple of the cheek. Voluma also pulls the lower face upward, addressing wrinkles in the lower face and loss of jawline definition.
Juvederm is also a dermal filler which works best in the lower face. It can be used to treat what we call the naso-labial folds. These are the wrinkles that go from the sides of the nose to the corners of the mouth. In some people this wrinkle continues all the way down to the jawline, forming what is called marionette lines, which can also be treated with Juvederm. Juvederm can also be used to provide volume and fullness to the lips, another common concern.
Volbella is a new filler in the Juvederm family designed for subtle enhancement of the lips. Volbella can fill in fine lines on or around the lips and help define the lip borders. Volbella can be combined with Juvederm in the lips for further enhancement.
Kybella is the first and thus-far only FDA-approved non-surgical treatment for the double chin. Kybella dissolves the fat in this area and improves the appearance of the double chin and at the same time improves the definition of the jawline.
The liquid facelift can also be combined with other treatments for even more enhancement. For example, a fractional CO2 laser resurfacing treatment will address the color and texture of the skin. A good skin care regimen such as products from the A E Skin line will enhance the results of all of these treatments.

Kybella is the first and thus-far only FDA-approved, non-surgical, injectable treatment for the double chin. What people love is that Kybella is permanent and maintenance treatments are not required as they are with most other treatments such as Botox, Juvederm, Restylane, and Voluma. The launch of Kybella was in 2016. This year we are planning to expand the use of Kybella to other areas such as the bra fat.
Volbella is a new filler in the Juvederm family released in October 2016. Volbella is designed for subtle enhancement of the lips. Specifically, it works well for vertical lines on or around the lips. Volbella is also used to shape and enhance the borders of the lips. This year we will expand the use of Volbella in the lips by combining it with Juvederm. Juvederm is used to volumize the lips and Volbella is used to perfect the finishing touches. What people love about Volbella is that, like Voluma, it uses Vy-Cross technology which allows patients to enjoy their results for a full year.
Voluma is a dermal filler used to restore lost volume in the mid-face. Patients enjoy the results of their Voluma treatment for up to two years with optimal correction. Voluma is designed to be placed in the upper cheek, giving the face a lift without surgery. We are also using Voluma to restore volume in the temples, which occurs in many people over time. Voluma can be combined with other treatments such as Botox, Juvederm, and Fractional CO2 Laser Resurfacing (see below) for even more rejuvenation of the skin.
While the face is the most common area for rejuvenation, other areas are being treated as well. The hands have become an increasingly popular area for rejuvenation. The Hand Rejuvenation treatment is a combination treatment designed to address multiple aspects of aging in the hands in a single treatment. Although recovery time can be one to two weeks, the results of a Fractional CO2 Laser Resurfacing treatment are so impressive that it’s worth it.
