One of the most commonly neglected areas for facial rejuvenation is the hollowing in the temples. Voluma is a filler that can be used to correct hollow temples.
Over time there are many changes that occur in the face. These include wrinkle formation, loosening of the skin, and dark spots. Additionally, bony structures change and fat is lost. These changes are very prominent and common in the temples. The bones move inward and the fat pad in the area is lost. This occurs more frequently in thin people who exercise a lot, leading to an appearance of a skeleton in the upper face.
These changes can easily be corrected with filler treatments. Fillers are products that are used to restore lost volume in the face and other areas on the body. Dr. Alex finds that Voluma is the best product in this area. Voluma is a dermal filler in the Juvederm family, the most popular family of fillers in the United States. The active ingredient in Voluma is hyaluronic acid, a sugar that’s found in the skin naturally. Hyaluronic acid binds a thousand times its weight in water so it draws water into the skin providing volume. This helps fill the hollows and provide a natural and healthy appearance.
In contrast to other hyaluronic acid products, Voluma has a high level of hyaluronic acid. This is perfect for placement on bones such as in the temples or upper cheeks. The treatment for the temples is very easy because there is only a single injection point on each side as is seen in the video demonstration shown here. An immediately lift is seen in the area.

An important advantage of Voluma is that it is safe in the event of side effects. The most concerning side effects occur when a filler is inadvertently placed into an artery, which blocks blood supply to the area. Hyaluronic acid products can be dissolved which allows us to easily treat these types of side effects. On the other hand, non-hyaluronic acid fillers such as Radiesse, Sculptra, and Bellafil cannot be reversed and therefore such side effects are much more dangerous.
Swelling is normal after a Voluma treatment to treat hollow temples. It may also be difficult to chew for a few days because the swelling puts pressure on a muscle in the area that we use to bite. Dr. Alex recommend avoiding heat exposure, alcohol consumption, and strenuous exercise for 24 hours after a Voluma treatment. To reduce swelling after a Voluma treatment Dr. Alex recommends applying ice packs on and off for five minutes at a time as much as possible for the first few days. Sleeping on an extra pillow will reduce the amount of blood brought to the skin overnight and decreases the swelling. Also, avoiding salty foods will prevent your body from retaining water and therefore decrease swelling. Most people require two syringes of Voluma per side. Full results are seen two weeks after the Voluma treatment once the swelling has gone down. The best thing about a Voluma treatment for hollow temples is that you can enjoy the results for two years or more.
Some of the most common concerns for our patients include uneven color such as dark spots or redness and irregular texture such as fine lines, wrinkles, enlarged pores, rough skin, and acne scars. The #nofilter treatment is an easy treatment to address all of these concerns.
The #nofilter treatment is a combination treatment designed by Dr. Alex to address multiple concerns at the same time with minimal down time. The name #nofilter treatment comes from the popular filters used on cellular telephones when taking pictures. Many people use these filters to make themselves look better in the picture. The idea is that you will no longer need to use these filters after your series of #nofilter treatments.
A number of factors contribute to the changes we see in our skin. The most important is sun damage, which leads to sun spots, redness, and broken capillaries. The sun also damages collagen in the skin. Collagen is the protein that keeps the skin firm, smooth, and youthful in appearance. Over time collagen is lost, leading to wrinkles and saggy skin. Also, collagen can be damaged from acne breakouts, especially if you pick at them. This causes acne scars.
The first step in the #nofilter treatment is a Forever Young BBL IPL photofacial. This step uses bright light to target and get rid of dark spots and redness. The first step takes just a few minutes and feels like a hot rubber band. After that we remove your eye protection and the gel from your skin. Before the second step in the #nofilter treatment we provide a different eye protection; this is a metal eye shield. During the second step of the #nofilter treatment gel is not needed but we continune to blos the cold air on your skin to keep you comfortable. The second step of the #nofilter treatment feels like a pins-and-needles sensation and also takes only a few minutes. After the #nofilter treatment we remove your eye protection and clean the skin. Your skin will feel hot for a few hours. We apply a hydrating mask immediately after the #nofilter treatment to calm down your skin and keep you comfortable. You can apply ice packs to your skin at home after your #nofilter treatment.
The best thing about the #nofilter treatment is that there is minimal down time. You can apply makeup and go to work the next day. Dr. Alex recommends a series of six #nofilter treatment done about once a month. After that Dr. Alex recommends a single #nofilter treatment every three months to keep your skin looking great. Lasers are routinely used to address cosmetic concerns. Now we have a new application: pain.
What laser is used for pain?
We use the Sciton SkinTyte II laser to treat pain. This uses gentle micro-pulses of red and infrared light. The light heats up the deeper layers of the skin. For cosmetic purposes, this stimulates the skin to form its own natural, new, fresh and healthy collagen to tighten and rejuvenate the skin. It also heats deeper tissues such as muscles, fat, and bone. When these tissues are injured the area becomes inflamed and swollen. The swelling and inflammation puts pressure on the surrounding areas which causes pain. The deep heating from the laser pain treatment stimulates blood flow to the area. This allows for faster healing and a decrease in the inflammation, and this decreases the pain.
How long is the laser pain treatment?
The laser pain treatment takes about five to ten minutes depending on the size of the area. The treatment is very comfortable and you won’t need any numbing cream. The laser slowly and gently heats the area. In general, a significant improvement in the level of the pain is noticed immediately after the treatment. It also decreases the requirement for oral pain medications. When joints are treated, people notice an increase in range of motion. For example, when a should is treated, after the treatment patients are able to lift their arms higher than they did before the treatment. When knees are treated patients are able to walk without pain.
How many laser pain treatments do I need?
Depending on the person, the pain, and the possible injury multiple treatments may be required. For minor soreness from exercise, sometimes a single session will be enough. For injuries often times a series of six weekly treatments are recommended. For chronic pain most people start with a series of six weekly treatments followed by one treatment every one to three months.
On July 14, 2018 Dr. Alex attended the Ultimate Aesthetics Training Workshop at the Luxe hotel in Beverly Hills, CA. This is another educational event designed to teach physicians in the cosmetic field about new products, devices, and techniques. Organized by Angela O’Mara and Giles Raine of The Professional Image, this workshop features many industry-leading doctors in the field to share their knowledge.
The day started with breakfast on the patio with various exhibitors such as Allergan, makers of Botox, Juvederm, Voluma, Vollure, Volbella, Kybella, and Latisse. Allergan is now distributing new brochures which feature the journeys of individual patients. These individuals had their treatments in stages, meaning they initially received one or two syringes of product and then more product was added over several sessions weeks to months later. This allows people to learn about what the treatments are like and how improvement is seen slowly over time when treatments are done in stages.
The day continued with a variety of lectures by distinguished doctors. Dr. John Vartanian discussed best practices to use Botox and fillers such as Voluma, Vollure, and Volbella for facial enhancement. Dr. Anil Shah reviewed a variety of pearls to help a practice. This included some things that are already incorporated into our practice such as the use of the beauty bar, a vibrating distractor placed on the skin during injectable and other treatments to maximize patients’ comfort. Dr. Alexander Rivkin of Westside Aesthetics spoke about techniques to feminize the face and the non-surgical nose job, a treatment which he pioneered. The session concluded with Dr. Kian Karimi who provided an overview of cannulas, threads, and PRP. After another break with the exhibitors, the day concluded with live demonstrations of injectables such as Botox, Voluma, Vollure, and Volbella.
The Ultimate Aesthetic Training Workshop is another outstanding educational opportunity that Dr. Alex takes advantage of regularly. One of the common side effects with treatments involving needle injections is bruising. It is impossible to eliminate the risk of bruising. However, we now have an anti-bruise patch that reduces the risk of bruises significantly.
Why do bruises occur?
Bruises are seen when a needle goes through a blood vessel and the blood leaks into the skin. Bruises can also be caused by physical trauma such as injuries. Bruises come in a variety of colors including red, blue, and purple. They can turn different colors such as yellow or brown as they mature and slowly go away. Injectable treatments such as Botox, fillers (Voluma, Vollure, and Volbella), and Kybella can lead to bruises. With filler treatments, needles are injected deeper and bruises can be larger. Bruises usually resolve on their own in one to two weeks.
Can bruises be treated?
We can treat bruises with a laser. The light from the laser hits the bruise and makes it go away faster. After a laser bruise treatment, bruises usually resolve in one to three days, but still could last a full two weeks. Also, sometimes they get darker before they get lighter. The laser bruise treatment is very easy. A demonstration is shown below. A small amount of gel is applied to the skin and the laser treatment feels like a small snap on the skin. A few pulses are used for each small area and the treatment itself usually takes less than one minute. Often times a change in color can be seen immediately.

What is the anti-bruise patch?
The anti-bruise patch is a new and innovative technology that is simple to use and significantly reduces the risk of bruising after injectable treatments and after surgery. It also makes bruises resolve more quickly and reduces swelling in the skin. The patch is clear and sticks to the skin. For most treatments two patches are used, one per side of the face. They are applied on the skin immediately after a treatment and left on for six hours. Then the patches are removed and replaced with another set of patches for another six hours; this can be done at bedtime and left on overnight while asleep. Ensure that the skin is clean and dry when applying the patches because any dirt or products on the skin will decrease the anti-bruising effects.
How does the anti-bruise patch work?
The anti-bruise patch contains very high doses of arnica and ledum, natural ingredients that have been used for years. These ingredients reduce swelling, bruising, pain, and inflammation. Arnica is available over-the-counter in both topical and oral forms. However, the concentration of arnica in the anti-bruise patch is 50 times stronger than that available over-the-counter. Each pair of patches comes in a sterile sleeve that can be kept at room temperature without the need for refrigeration.
How can I reduce the risk of bruising after my treatment?
With any injection there is always the risk for bruising. Dr. Alex recommends avoiding injectable treatments within two weeks of special events where you want to look your best. Also, avoid heat exposure, alcohol consumption, and strenuous exercise for 24 hours after a treatment because these activities bring blood to the skin and can increase the risk of bruising. It is also recommended to avoid medications that thin the blood such as aspirin as well as natural blood thinners like turmeric, ginger, cinnamon, cayenne peppers, and vitamin E for a few days before and after and injectable treatment. Tell Dr. Alex if you are taking prescription-strength blood thinners such as coumadin or Plavix.
Myth: Botox is not safe.
The safety record of Botox is well established. Botox has been used for over 20 years for a variety of treatments with minimal side effects. Botox is amongst the safest treatments for treating facial wrinkles.
Myth: There are creams that work better than Botox.
Topical creams work on the surface of the face. Botox works on the muscles under the skin which cause the deeper wrinkles to form. Once the muscles relax, the skin overlying it can smooth out. When used in combination with A E Skin products such as the Brightening Cream, better results are seen because treatment is directed at both the superficial and deep wrinkles.
Myth: Botox is only for the rich and famous.
According to the Aesthetic Surgery Education Research Foundation Botox Cosmetic Use Survey of 1048 patients (2005) the most common users of Botox are working mothers aged 40-55; and those with an annual household income of less than $ 100,000 are more likely to use Botox than those with an annual household income of more than $ 150,000. And with the Allergan Brilliant Distinctions Program, patients are able to save on every Botox treatment.
Myth: Botox is only for old people.
A common misconception is that Botox is only for those who are in their 60's or 70's. On the contrary, we have a lot of patients in their 20's who receive Botox. This is either for prevention or for those who have expressive faces. I find that the younger one is when one starts receiving Botox treatments the more signs of aging will be delayed. In other words, Botox can be used to prevent wrinkles from either appearing in the first place or from getting deeper over time.
Myth: Botox isn't for men.
A common misconception is that Botox is only for women. On the contrary, similar to women, many men are concerned about their appearance and want to look their best. In the past several years Botox and other cosmetic treatments such as fillers and laser rejuvenation have become more and more popular in the male population. It is also important to understand that men have different facial features than women and a masculine appearance is desired. Therefore, an expert injector should understand the anatomic differences between the genders and know how to use the product to produce desired results. One example is the position and shape of the eyebrow. While the ideal eyebrow for women is high with an arch, men have lower set brows that are more horizontal. It is critical that the product is placed in the correct positions on the face to avoid feminizing the eyebrows.
Myth: It is safe to have a Botox treatment during pregnancy or when breast feeding.
Although Botox is one of the safest treatments offered, there are some situations where it is not appropriate. The safety and efficacy of Botox treatments have not been determined in women who are pregnant, trying to become pregnant, or breast feeding. There is also no evidence that Botox crosses the placental barrier, but such studies have not been published. While some physicians are comfortable treating women who are pregnant, trying to become pregnant, or breast feeding, in the absence of data we err on the side of caution and recommend that my patients avoid Botox and other such cosmetic treatments until they are no longer breast feeding.
Myth: Botox can only be used in the upper face.
Although Botox is most frequently used in the upper face, it can also be used in various areas in the lower face. The nasalis muscle on the bridge of the nose is responsible for the bunny lines that are sometimes seen there when one frowns or smiles. Botox can be placed in the orbicularis oris muscle around the lips to provide a pout to the lips and reduce the appearance of vertical lines around the lips sometimes called smoker’s lines (although they are seen in people who do not smoke as well). Botox can be placed in the depressor anguli oris (DAO) muscle to elevate downturned corners of the mouth which sometimes produce a sad face appearance. Botox can be placed in the mentalis muscle on the chin to address a cobblestone appearance or dimpling on the chin. Botox can be placed in the masseter muscle in the lower face to contour the lower face and address pain associated with TMJ. Botox can be placed in the levator labii superioris alequae nasii (LLSAN) muscle located just beside the nose on either side to address what is called the gummy smile. The gummy smile is seen in some people whose upper lip elevates so high when they smile that the upper gums are seen in addition to their upper teeth. Botox can be placed in a vestigial muscle (seen in some people) called the depressor septi nasii to prevent the tip of the nose from moving downward when smiling. Botox can be placed just under the jawline in what is called the Nefertiti lift to relax the platysma muscle which pulls down on the lower face, thereby improving jawline definition. When placed lower in the neck Botox can address vertical neck bands.
Myth: Botox will leave me with a frozen face.
Many people are concerned about how their appearance will change after a Botox treatment. This is understandable given the amount of both positive and negative publicity there has been around Botox and other cosmetic treatments. Although results are visible, treatment with Botox does not make you look like you had work done. The muscles causing the frown lines are relaxed, so you can still have expressions, just without the wrinkles. When done properly, Botox will provide a natural appearance that makes a person look more youthful and rested. With the increase in popularity of Botox treatments many offices now provide such treatments. It is important to find an office where the injector has experience and expertise in Botox injections. Look for experience, before and after photographs, and testimonials.
Myth: If I stop using Botox, my face will look different than before.
A common misconception is that if Botox treatments are discontinued the wrinkles will look worse than they did prior to starting the treatments. When Botox treatments are discontinued the treated muscles are no longer relaxed and they are able to contract normally as they did prior to starting the Botox treatments. When this occurs, the wrinkles will begin to reappear and will look as they did prior to Botox treatments. This usually takes three to four months.
Myth: Botox treatments are painful.
One of the most common questions regarding Botox is whether the treatment is painful. We use a painless technique to maximize our patients comfort during the treatment. This starts by reconstituting the Botox with preserved saline, which contains benzyl alcohol, to decrease the discomfort with the injection. We use a tiny needle, similar to the size of the needle used for TB test on the forearm. During the Botox treatment, we provide a relaxing environment by playing a calming video with gentle music. We provide stress balls to our patients to hold during their Botox treatment. We apply cold air on the area which serves to numb the skin. We also use the Beauty Bar, which is a vibration device which distracts the patient from the injection of Botox itself. Finally, with experience, we have developed a technique to inject to minimize pain. This includes pinching the area, pushing the skin into the needle (as opposed to pushing the needle into the skin), using a slight rotation, and using gentle pressure. Most patients are surprised at how comfortable the Botox treatment is.
Painless Botox Treatment Demonstration
Myth: Botox can cause Botulilsm.
Botox was originally obtained from the bacterium Clostridium botulinum, the bacteria that causes botulism. These bacteria can be found in certain foods that are not properly preserved. When it is ingested the bacteria multiplies and spreads through the bloodstream releasing a protein called botulinum toxin, the active ingredient in Botox. As the bacteria multiply more and more of the toxin is released. The botulinum toxin attaches to the nerves on large muscles such as those responsible for breathing or those in the limbs. This makes it difficult to breath and perform activities such as walking. The live bacteria are required to multiply and continue releasing large amounts of the toxin. In the absence of the bacteria, as when Botox is used for cosmetic purposes, more toxin is not produced. Also, the number of units used for cosmetic Botox treatments is miniscule compared to the amount required to produce effects such as difficulty breathing. Finally, the minute amount of Botox used in cosmetic treatments is not enough to pass into the bloodstream and spread to other muscles, as evidenced by the lack of systemic side effects.

What should I do after the treatment?
After the treatment, carefully follow the following aftercare instructions: Please avoid heat, alcohol, and exercise for 24 hours after the treatment. Heat refers to going in the hot sun outdoors, if it were to be hot outside, but also any hot environments indoors such as a hot bath, sauna, or steam room. Also avoid hot or spicy foods or drinks such as soup, hot coffee, or hot tea because all of these things will bring blood to the skin and can cause problems such as redness, blisters, or discoloration. Please avoid makeup for 24 hours after the treatment because the skin will be sensitive. You may be red for 24 to 48 hours after the treatment. Redness is normal, don’t be alarmed. Feel free to apply cold compresses to the skin to keep yourself comfortable if necessary. Sometimes the skin gets darker before it gets lighter. This is normal. It could take a couple of weeks and prolonged discoloration can be treated if necessary. Please let us know if this happens. Be sure to use sunscreen every day.
When will I see results?
Improvement is seen slowly over time. For optimal results an initial series of six monthly treatments is recommended followed by one maintenance treatment every three months thereafter.

It is a fact that we all age, but there are many things we can do so that we don’t look like we are aging. Signs of aging include fine lines, wrinkles, redness, broken capillaries, and uneven color. In this post we discuss what can be done to reduce and prevent the appearance of signs of aging.
Protecting the Skin
The single most important thing a person can do to prevent signs of aging is to protect herself from sun damage. The sun damages the skin in many ways. Sun exposure leads to fine lines, wrinkles, leathery skin, broken capillaries, sun spots, uneven color, and skin cancer. The skin can be protected from the sun in many ways. First, the skin on the body can be easily protected with clothing. On the face and other areas that are exposed, Dr. Alex recommends applying sunscreen daily. The sunscreen should be at least SPF 30 with broad spectrum (or UVA and UVB) protection. Sunscreen should be re-applied every one to two hours if outdoors for prolonged periods. Remember that when driving in the car you are exposed to sun through the windows and windshield. Other things to do include wearing a broad-brimmed hat, staying in the shade, and avoiding peak sunlight hours in the middle of the day. In addition to sunscreen, it is also important to keep the face clean and apply moisturizer twice daily to retain the water in the skin necessary for all of the skin’s important functions. Mature skin tends to be drier and heavier moisturizers work better.
Exfoliation of the skin removes the dead skin cell from the surface. With age, the skin cell growth cycle slows down, which leads to the buildup of dead skin cells on the surface. Exfoliation will remove this layer, which leads to smooth, bright, and glowing skin. Exfoliation can be done with scrubs, Clarisonic or other brushes, daily application of glycolic or salicylic acid, chemical peels, and laser resurfacing.
Laser treatments can help address a number of concerns including fine lines, wrinkles, dark spots, uneven color, redness, and broken capillaries. Such treatments can be started at any age and are tailored for the individual.
Botox and Fillers
Botox and fillers are injectable treatments that can help slow down signs of aging. By relaxing muscles, Botox addresses wrinkles associated with facial expressions such as frown lines and crow’s feet. Fillers work to restore lost volume in various areas such as the temples, cheeks, tear troughs, laugh lines, lips, and chin. By using small amounts of products a natural appearance can be achieved.
The IPL photofacial is one of our most popular treatments because it can treat a variety of concerns such as uneven color, dark spots, sun spots, redness, and broken capillaries. Although the treatment can provide outstanding results side effects are possible if the device is not used appropriately.
How does an IPL photofacial work?
The IPL photofacial works by using bright light to target and destroy unwanted pigment and redness in the skin. As the light passes through the skin it is absorbed by the pigment and blood. This heats up the pigment and blood and destroys it. There are many settings that can be adjusted to customize the treatment for each person during an IPL photofacial treatment. Those with darker skin have a higher risk of complications and therefore more conservative settings must be used for each IPL photofacial treatment. In such cases more IPL photofacial treatments are performed and results are seen more slowly. The therapeutic window for the IPL photofacial is narrow. The therapeutic window is the difference between the minimum settings required for improvement and the settings which cause damage. The Forever Young BBL IPL photofacial device by Sciton is an outstanding device that provides superior results for all skin types because of its versatility. Dr. Alex is able to adjust many different settings on the Forever Young BBL IPL photofacial device by Sciton for each person. Like most IPL devices on the market, the fluence, or energy settings, can be adjusted on the Forever Young BBL IPL photofacial device by Sciton. However, unlike other devices, the Forever Young BBL IPL photofacial device by Sciton has a number of features that make it the best device available. For example, the filter is easily changed to change the color of light used. This is important because lower wavelengths (colors) can be more aggressive and are therefore avoided in those with darker skin types.
