For your convenience, many forms are available here. These forms can be downloaded and printed. To save time, please complete the paperwork and bring it to your appointment. Patients are required to sign appropriate consent forms prior to each treatment. Consent forms describe the risks, benefits, and alternatives to the treatment. A signature acknowledges an understanding of the risks, acknowledges that the patient would like to undergo the treatment given the risks, and gives us permission to perform the treatment. Since these acknowledgements as well as a person's medical history may change, signing a consent form is required prior to each treatment. We would never do a treatment without a patient's permission and we never do treatments without a consent form signed on the day of the treatment. Our patients' safety is our number one priority. Thank you for your understanding and cooperation.
A medical history form is required for all new patients. Please download the form and fill it out prior to your first appointment. Returning patients do not need to complete this form again.
